This form was designed for academic department use/processing and will not be processed in either the University Registrar’s Office or the 1-Stop Service Center. (The only exception will be adding a course for “Audit” credit – those requests will continue to be processed only in the Office of the University Registrar.) - Student Information (used to schedule lessons, gain access to practice rooms, and get informational e-mails)
